Question 1 · Quality Leadership And Integration
A quality director is facilitating a SWOT analysis to inform the organization's multiyear quality plan. Which finding should be placed in the threats quadrant?
Answer: D. A competing health system is opening a freestanding emergency department within the primary service area
Threats are external factors outside the organization's control that could harm performance, such as a competitor entering the service area. High nurse turnover and an outdated tracking interface are internal weaknesses, and the stroke certification is an internal strength. Correctly separating internal from external factors is what makes a SWOT useful for strategy.
Question 2 · Teams
Four meetings into a project, members of an improvement team argue about the scope of the aim, question who is responsible for data collection, and challenge the facilitator's approach. Which stage of team development is this, and what should the facilitator do?
Answer: A. Storming; acknowledge the conflict and revisit the charter, ground rules, and roles
In Tuckman's model, storming is marked by conflict over goals, roles, and leadership. The facilitator should surface the conflict constructively and return the team to its charter, ground rules, and role clarity so it can move to norming. The other stages do not match the behaviors described, and stepping back or empowering the loudest voices would prolong the conflict.
Question 3 · Population Health And Care Transitions
A nonprofit hospital has just completed its community health needs assessment (CHNA), identifying diabetes, behavioral health access, and food insecurity as significant needs. What is required NEXT?
Answer: A. Adopt an implementation strategy describing how the hospital will address the identified needs
Tax-exempt hospitals must conduct a CHNA at least every three years, make it widely available, and adopt an implementation strategy that addresses the significant needs identified (or explains why a need will not be addressed). CMS does not approve CHNAs, annual repetition is not required, and the assessment must include input from the broader community, not only the hospital's own patients.
Question 4 · Statisticsforquality
An analysis of 24 months of data finds a correlation coefficient of r = 0.72 between monthly nurse overtime hours and inpatient falls. Which conclusion is MOST appropriate?
Answer: C. There is a strong positive association, but causation cannot be concluded from these data
An r of 0.72 indicates a strong positive linear association, but correlation does not establish cause; both variables may be driven by a third factor such as patient census or acuity. The coefficient is not a percentage of cases caused, it does not indicate a weak relationship, and it cannot be used to predict the effect of an intervention.
Question 5 · Safetyprinciplesandframeworks
A wrong-site surgery analysis finds that the site marking was omitted, the time-out was rushed during a staffing shortage, and the imaging displayed in the room was mislabeled. How does Reason's Swiss cheese model explain this event?
Answer: D. Weaknesses in several defensive layers lined up, allowing the error to reach the patient
Reason's model depicts defenses as slices of cheese with holes (active failures and latent conditions); harm occurs when holes in multiple layers align. The model argues against attributing the event to one person or one factor, and it implies that strengthening each layer makes such events preventable.
Question 6 · Regulatory And Accreditation
A hospital seeks an accreditor with deeming authority that conducts annual surveys and integrates accreditation with an ISO 9001 quality management system. Which option meets these criteria?
Answer: A. DNV NIAHO accreditation
DNV's NIAHO program has CMS deeming authority, surveys hospitals annually, and requires compliance with or progress toward ISO 9001. Magnet is a nursing excellence recognition and the Baldrige award is a performance excellence recognition, neither of which confers deemed status; Joint Commission hospital surveys occur on an approximately three-year cycle and do not incorporate ISO 9001.
Question 7 · Qualitygovernanceandculture
Frontline staff tell the quality professional that improvement ideas raised in unit meetings 'just disappear.' Which leadership practice would MOST strengthen the culture of quality?
Answer: A. Tracking suggestions visibly and reporting back on the decisions made
Closing the loop, by visibly tracking ideas and explaining what was done and why, shows staff their input matters and builds engagement in improvement. Routing ideas through formal committees and manager screening adds delay and filtering, and rewarding volume encourages quantity rather than meaningful participation.
Question 8 · Monitoringtimelinessandprojectdeliverables
A perioperative team is monitoring first-case on-time starts (FCOTS) against a target of 80%. Based on the data in the table, which conclusion is BEST supported for the four-week period? 1 40 26 2 38 27 3 42 33 4 40 34
Answer: C. 75.0% overall; below target, although week 4 exceeded it
Overall FCOTS = (26 + 27 + 33 + 34) / (40 + 38 + 42 + 40) = 120 / 160 = 75.0%, below the 80% target. Weekly rates rose from 65.0% to 71.1%, 78.6%, and then 85.0% in week 4, so the target was exceeded in week 4 and performance is improving, but one week does not demonstrate sustained attainment.
Question 9 · Measurementanddatacollection
A quality team is auditing pain reassessment compliance. Because records are grouped by unit, the team randomly selects 4 of the hospital's 12 inpatient units and audits every eligible record on those 4 units. Which sampling method is being used?
Answer: A. Cluster sampling
Cluster sampling randomly selects whole groups (here, units) and includes all members of the selected groups. Stratified sampling would draw a random sample from every unit, simple random sampling would give every record in the hospital an equal chance of selection, and convenience sampling uses records that are easiest to obtain without randomization.
Question 10 · Patient Safety
Before closing an incision, a circulating nurse twice tells the surgeon that the sponge count is incorrect. The surgeon acknowledges neither concern and begins to close. Using TeamSTEPPS tools, what should the nurse do NEXT?
Answer: C. State 'This is a safety issue' and escalate through the chain of command
The two-challenge rule directs a team member whose concern has been voiced twice and disregarded to take stronger action, including stopping the process and using the chain of command; CUS escalates to 'This is a safety issue.' Allowing closure, repeating the same message, or reporting after the fact fails to prevent a retained foreign object.
