Question 1 · Practicesettingsandthepharmacyteam
A technician starts a job at a pharmacy that has no walk-in customers. It fills medications for nursing home residents in multi-day blister cards, delivers on a set schedule, and a consultant pharmacist reviews each resident's drug regimen monthly. Which practice setting is this?
Answer: C. Long-term care pharmacy
Long-term care pharmacies are typically closed-door operations that serve nursing homes and assisted living facilities, using blister cards or unit-dose packaging and monthly consultant pharmacist drug regimen reviews. A mail-order pharmacy also ships without walk-in traffic, but it serves individual outpatients rather than facility residents. Specialty and home infusion pharmacies focus on high- cost biologics and IV therapy, not routine facility cycle fills.
Question 2 · Hipaaandprotectionofpatientinformation
A customer calls to pay for a prescription over the phone so a family member can pick it up, and reads out her credit card number. How should the technician handle the card information?
Answer: A. Enter it directly into the payment system without writing it down
Credit card numbers, Social Security numbers, and similar personally identifiable information must be protected like PHI: collected only when needed, entered into secure systems, and never left on paper or in unsecured notes. Writing the number on a bag exposes it to everyone who handles the bag, free-text profile notes are not a secure payment vault, and email is an unsecured channel.
Question 3 · Medication and Patient Safety and Quality Assurance
The pharmacist recommends a less-sedating, second- generation oral antihistamine for a customer's seasonal allergies and asks the technician to bring it from the aisle. Which product should the technician bring?
Answer: C. Claritin (loratadine)
Loratadine is a second-generation antihistamine that causes little drowsiness at usual doses, as do cetirizine (Zyrtec) and fexofenadine (Allegra). Diphenhydramine, chlorpheniramine, and doxylamine are first-generation antihistamines that cross into the brain and cause marked sedation; doxylamine is sold specifically as a sleep aid.
Question 4 · Preparing,Labeling,Anddispensingprescriptions
A technician is putting away an order from the wholesaler. Which item must be placed in the refrigerator (2 °C to 8 °C) until it is dispensed?
Answer: B. Unopened Lantus (insulin glargine) pens
Unopened insulin is stored in the refrigerator at 2 °C to 8 °C (36 °F to 46 °F) and must never be frozen; once in use, a pen may be kept at room temperature for a limited time. Levothyroxine, atorvastatin, and amoxicillin capsules are stored at controlled room temperature, 20 °C to 25 °C (68 °F to 77 °F).
Question 5 · Sterilecompounding
A technician is reconstituting a powdered antibiotic for a patient in the neonatal intensive care unit. The package insert lists sterile water for injection as the diluent. Which diluent should the technician select?
Answer: A. Preservative-free sterile water for injection
The diluent named in the package insert is used, and neonates must not receive benzyl alcohol, a preservative linked to "gasping syndrome." Both bacteriostatic products contain benzyl alcohol, and lactated Ringer's is not the labeled diluent and may be incompatible with the drug.
Question 6 · Practicesettingsandthepharmacyteam
A pharmacy dispenses high-cost injectable biologics for conditions such as rheumatoid arthritis and multiple sclerosis, coordinates prior authorizations and manufacturer assistance, ships refrigerated packages, and calls patients monthly to monitor adherence. Which practice setting is this?
Answer: D. Specialty pharmacy
Specialty pharmacies manage expensive, complex therapies such as biologics, including prior authorization, financial assistance, cold- chain shipping, and ongoing patient monitoring. Nuclear pharmacies prepare radiopharmaceuticals, compounding pharmacies prepare customized preparations, and long-term care pharmacies serve facility residents.
Question 7 · Hipaaandprotectionofpatientinformation
A technician realizes she has just faxed a patient's medication profile to the wrong physician office. What should she do FIRST?
Answer: B. Report it to the pharmacy's privacy officer or supervisor
A misdirected fax is a possible breach of unsecured PHI, so the privacy officer must evaluate it, document it, and decide whether the patient and HHS must be notified. Staff report it immediately rather than deciding on their own that no harm occurred. Waiting or clearing the fax log hides the incident and violates breach policies.
Question 8 · Drugs and Drug Therapy
A patient updating his profile lists his medications by brand name. Which brand/generic pair is correctly matched?
Answer: C. Lexapro / escitalopram
Lexapro is escitalopram, an SSRI antidepressant. Cymbalta is duloxetine (venlafaxine is Effexor XR), Wellbutrin is bupropion (buspirone is an anxiolytic once sold as BuSpar), and Seroquel is quetiapine (olanzapine is Zyprexa).
Question 9 · Preparing,Labeling,Anddispensingprescriptions
A mail-order pharmacy is shipping insulin pens to a patient during hot weather. Which packing method is appropriate?
Answer: A. An insulated shipper with cold packs and a barrier so the pens do not touch the packs
Insulin must stay between 2 °C and 8 °C and must not freeze, so it is packed in a validated insulated container with refrigerant packs separated from the product. Dry ice, or frozen packs touching the pens, can freeze the insulin and destroy its potency. An uninsulated envelope can overheat in hot weather, even with overnight delivery.
Question 10 · Sterilecompounding
During final inspection, a technician holds a compounded IV bag against light and dark backgrounds and sees several small white particles floating in the solution. What should be done?
Answer: C. Do not release the bag; set it aside, notify the pharmacist, and document the problem
Visible particulates, cloudiness, leaks, or discoloration mean the CSP fails final inspection; it is not dispensed, the pharmacist is informed, and the event is documented so the CSP can be remade and the cause investigated. Shaking or waiting does not remove the particles, and passing the problem to the nurse does not make the product safe.
